Care gap

A care gap is a piece of recommended clinical care that a patient hasn't yet received, such as an overdue monitoring test or a missed review, identified by comparing the patient's record against expected criteria for their condition.

Care gaps can only be identified automatically if the underlying diagnoses and monitoring data are coded consistently. In a free-text record, finding them requires someone to manually review each patient's notes.
