Sending a Questionnaire to a Patient

You can send any questionnaire — organisation-built or system-provided — directly to a patient via the message composer.

Steps

  1. Open the message composer (see How to Send a Patient a Message)
  2. Click + Add link in the toolbar below the message
  3. Choose Questionnaire from the "Add to message" menu
  4. Find the ones you want:
    • Search by name, or filter by category using the pills at the top
    • Organisation questionnaires — custom forms your team has built
    • System questionnaires — built-in validated instruments (e.g. PHQ-9, GAD-7)
  5. Click each questionnaire you want to include. The footer keeps a count.
  6. Click Insert to attach them to your message

Each questionnaire appears as a block in your message, which you can remove before sending.

Assigning Responses

On the questionnaire block in your message, click Edit links & assignment. Each questionnaire there has an Assign responses to dropdown - pick the team member who should receive the completed form, or leave it Unassigned if it does not need routing to anyone in particular. Click Done when you have finished.

What the Patient Sees

When the patient clicks the secure link:

  1. They verify their identity
  2. The questionnaire opens in the patient portal
  3. They complete and submit it
  4. Responses are recorded in the patient's record and routed to the assigned team member

Tips

  • You can attach more than one questionnaire to the same message
  • You can include questionnaires alongside documents, a booking link and a reply in the same message — they all fold into one secure link for the patient
  • The patient can complete the questionnaire on any device — it's fully responsive
