How to Amend a Consultation

Once a consultation is finalised, it becomes an immutable clinical record. To make changes, you must use the amendment process, which records what was changed and why.

Starting an Amendment

Open the finalised consultation from the consultation history and click Amend. The consultation reopens in amendment mode.

Amendment Reason

Before you can make changes, a dialog asks for the reason for amendment. This is a medico-legal requirement. Enter a clear explanation of why the consultation needs to be changed.

What You Can Change

In amendment mode, you can update:

  • The responsible clinician
  • Clinical content - problems, notes, observations, medications, referrals, and all other clinical data

The consultation metadata (original date and time) is preserved.

Amendment Workflow

The wizard steps change in amendment mode:

  1. Edit - make your changes to the clinical record
  2. Review - preview the amended consultation
  3. Complete - finalise the amendment

Amendment History

Every amendment is recorded in the consultation's audit trail. The amendment history shows:

  • The original version of the consultation
  • Each amendment with:
    • Timestamp of when the amendment was made
    • Who made the amendment
    • The reason provided
    • What was changed

This creates a complete, traceable history of all modifications.

Tip: Always provide a specific, clinical reason for the amendment. Vague reasons like "correction" are less useful for audit purposes than "Added observation values recorded during examination but omitted from initial documentation".
