Weight Management Programme

The weight management programme provides a structured pathway for patients on GLP-1 or other weight management treatments. It tracks weight, BMI, safety indicators, and biomarkers from the point of enrolment.

Enabling the Programme

Navigate to App Store > Care Programmes > Weight Management and toggle it on.

Questionnaires

The programme includes three structured questionnaires:

Onboarding Assessment

Completed when a patient first joins the programme. Covers:

  • Height and weight measurements (saved as observations)
  • Previous weight management history (diet, exercise, medications, surgery)
  • Eating behaviour screening (binge eating)
  • Comorbidities (type 2 diabetes, hypertension, sleep apnoea)
  • Safety screening for GLP-1 contraindications (pregnancy, pancreatitis, thyroid cancer family history)
  • Mental health screening

Red flags are raised for safety-critical answers and are highlighted on the dashboard.

Monthly Follow-up

For regular monitoring between appointments:

  • Current weight
  • Medication adherence and side effects (nausea, vomiting, diarrhoea, constipation, abdominal pain)
  • Safety and wellbeing checks

Safety Check-in

A rapid triage form for use between monthly reviews:

  • Screens for severe vomiting, severe abdominal pain, and pregnancy
  • Free-text field for patient concerns

Dashboard

After enrolment, the dashboard shows:

Summary cards:

  • Latest weight and BMI
  • Weight change since enrolment (kg)
  • Programme duration (days)
  • Episode and questionnaire counts

Weight trend chart -- weight and BMI plotted over time from enrolment

Biomarker trends -- tracked from pathology results:

  • HbA1c, ALT, Triglycerides, Total Cholesterol, HDL, LDL, Fasting Glucose

Safety & tolerability -- clinical findings recorded since enrolment

Outstanding work -- open recalls and episodes requiring attention

Programme timeline -- chronological log of all weight measurements, questionnaire submissions, clinical findings, episodes, and recalls

Programme Insights

The dashboard automatically generates advisory insights based on the patient's data:

Weight gain detected Latest weight is 0.5+ kg above previous measurement
Weight loss plateau Less than 1 kg change across 3+ measurements or over 6 weeks
Milestone achieved 5%% or 10%% weight loss from enrolment weight
Missed follow-up No follow-up questionnaire submitted within 6 weeks
Persistent side effects Same clinical finding recorded 2+ times
Repeated safety symptoms Same red-flag finding recorded 2+ times

Insights are advisory and ephemeral -- they appear based on current data and disappear when conditions change.

Tip: Order baseline bloods (HbA1c, liver function, lipid profile, fasting glucose) at enrolment so the biomarker trends have a starting point to track against.
