Care History and Episodes

Care History provides a complete chronological timeline of all clinical events. Episodes let you group related clinical activity into care pathways.

Care History

Navigate to Clinical Record > Care History to see a timeline of every clinical event recorded for the patient.

Each event is colour-coded by type:

Event Type Description
Observation Vital signs and measurements
Procedure Medical procedures performed
Social History Social factors and lifestyle
Allergy Allergy records
Medication Medication changes
Referral Referrals to other services
Problem Problem records
Clinical Finding Other clinical findings

Click any event to view its full details in a sidebar.

To record a standalone clinical finding, click the + button in the Care History view.

Episodes

Episodes group related clinical activity into care pathways. Navigate to the Episodes tab in the patient record.

Creating an Episode

  1. Click Create
  2. Set the episode details:
    • Title - describe the care pathway
    • Journey Type - the type of care programme
    • Priority - High, Medium, or Low
    • Assigned to - a specific user or team
    • Due date - optional target date
  3. Save the episode

Episode Statuses

  • Open - actively being worked on
  • Pending Patient - waiting for the patient
  • Awaiting Review - ready for clinician review
  • Closed - completed

Filtering Episodes

Use the filters to narrow the episode list:

  • Assignment - Unassigned, Mine, By user, or By team
  • Priority - High, Medium, or Low
  • Journey Type - filter by care pathway type
  • Show Closed Episodes - toggle to include completed episodes

Sort by Newest, Oldest, or Priority.

Tip: Use episodes to track multi-step care pathways such as chronic disease management or post-operative recovery, where several consultations and actions need to happen over time.
