Managing Problems and Diagnoses

Problems record a patient's medical conditions, diagnoses, and health concerns. They are SNOMED-coded and tracked by status and significance.

How to Get There

Open a patient record and navigate to Clinical Record > Problems.

Problem Statuses

Problems are grouped by their current status:

Status Badge Meaning
Active Red Current ongoing condition
Health Administration Grey Administrative status (e.g. screening, immunisation status)
Significant Past - Resolved but clinically significant (always visible)
Minor Past - Resolved minor conditions (visible in full Problems tab only)

Adding a Problem

  1. Click the + button in the Problems section
  2. Search for the condition using SNOMED terminology
  3. Set the fields:
    • Onset date - when the condition started
    • Significance - Significant or Minor (determines visibility in summaries)
    • Remain on record - how long to display after resolution: 7 days, 14 days, 28 days, 3 months, 6 months, 1 year, or indefinitely
  4. Save the problem

Managing Problems

Click any problem to view its details in a sidebar. From there you can:

  • Edit - update status, significance, or other details
  • Resolve - move from Active to Past
  • Delete - remove the problem entirely

Significance

Significant problems always appear in the patient summary and clinical overviews. Minor problems only appear in the full Problems tab. Use significance to control what clinicians see at a glance.

Tip: Mark long-term conditions as Significant and set "remain on record" to indefinitely so they are always visible in the patient summary.
