Problems record a patient's medical conditions, diagnoses, and health concerns. They are SNOMED-coded and tracked by status and significance.
Open a patient record and navigate to Clinical Record > Problems.
Problems are grouped by their current status:

| Active | Red | Current ongoing condition |
| Health Administration | Grey | Administrative status (e.g. screening, immunisation status) |
| Significant Past | - | Resolved but clinically significant (always visible) |
| Minor Past | - | Resolved minor conditions (visible in full Problems tab only) |
Click any problem to view its details in a sidebar. From there you can:
Significant problems always appear in the patient summary and clinical overviews. Minor problems only appear in the full Problems tab. Use significance to control what clinicians see at a glance.
Tip: Mark long-term conditions as Significant and set "remain on record" to indefinitely so they are always visible in the patient summary.
You can edit a problem directly from the patient record: open the problem and choose Edit to make changes in a slide-in panel, without navigating away. Every change to a problem is now recorded in a history timeline shown on the problem itself, so you can always see what was changed, who changed it, and when.
