By default, when you reauthorise a medication or create a prescription document, the prescribing/authorising clinician is locked to your own linked clinician profile — you can't prescribe on someone else's behalf. This keeps prescribing accountable to the person actually signed in.
Go to Configuration → Consultations → Preferences and toggle "Lock prescriber to the signed-in clinician".
If you see this when prescribing, your user account hasn't been linked to a clinician profile in this practice. Ask a practice admin to link you (in the team/clinician settings), then try again.
Reauthorising a medication requires the Create prescriptions permission. If you don't have it, the Reauthorise action is hidden.
