Complete a questionnaire on behalf of a patient

Sometimes a patient hasn't filled in a questionnaire before their appointment — for example you're taking their answers over the phone or at reception. You can now complete the form for them.

Before you start

The questionnaire must already be attached to the appointment — set as a pre- or post-booking questionnaire on the appointment type. If nothing is attached, the appointment has no Questionnaires section and there is nothing to complete here. You attach questionnaires on the appointment type (see Attaching Questionnaires to Appointment Types); you cannot add one from the appointment itself.

How to do it

  1. Open the appointment from the calendar or appointments list — this is the appointment's details panel, which is separate from the consultation.
  2. In the Questionnaires section, find the form. If the patient hasn't completed it, it shows “Patient hasn't filled this in” with a Complete button.
  3. Select Complete. The questionnaire opens with a banner confirming you're completing it on behalf of the patient.
  4. Enter the patient's answers and select Save responses.

What happens

  • The response is saved to the patient's record and attributed to you — not the patient.
  • The patient is not notified.
  • The questionnaire then shows “Staff-completed”, and the patient's activity timeline records that you completed it on their behalf.

Who can do this

You need the update patient permission. If you don't see the Complete button, ask your practice administrator about your permissions.
